About The Position

This is a PRN position in TRU Community Care's Inpatient Unit. The Registered Nurse (RN) is a professional caregiver who assumes the responsibility and accountability for a group of patients for a designated time frame. The RN renders professional care in accordance with the physician/provider treatment plan and the nursing process of assessment, diagnosis, planning, and implementation. This RN functions as a member of the interdisciplinary team. The RN addresses physical, psychosocial, and spiritual aspects of care and ensures patient needs are quickly met in holistic, un-fragmented care in collaboration with Social Work, Chaplain, Volunteers, and Physicians. The RN is a clinical practitioner who coordinates and implements patient care specific to the patient population served and ensures that compassionate, engaged care meets or exceeds quality standards. All care is provided in an efficient and safe manner consistent with the Colorado Board of Licensure and TRU Community Care standard of care. The RN utilizes nursing skills to determine the biophysical, psychosocial, environmental, self-care, cultural, ethnic, spiritual, and educational needs of the patient/family/significant other to develop, implement, and revise the plan of care. The RN performs frequent and accurate assessments, interprets and correlates finding, establishes plan of care, and implements short term goals. The RN observes patient progress, records observations, implements physician and nursing treatment plans, and evaluates outcomes. The RN recognizes the legal responsibilities as specified by law and adheres to those responsibilities.

Requirements

  • Must possess a current RN license in the state of Colorado
  • Must have a minimum of one year RN experience
  • Must successfully pass agency required background checks and drug screening
  • Requires travel to multiple work locations
  • Must comply with agency health screening and immunization requirements
  • Must possess CPR certification or be able to obtain within 60 days of hire.
  • Must be able to demonstrate computer competency

Nice To Haves

  • hospice or home health experience preferred

Responsibilities

  • Provides nursing care to meet the patient’s needs in accordance with the patient’s hospice plan of care.
  • Correctly identifies, communicates, and collaborates on patient problems/priorities/issues based upon assessment data.
  • Establishes measurable patient goals and communicates on-going plan of care appropriately to the interdisciplinary team (IDT).
  • Recognizes changes in the clinical status of the patient and reports findings to the appropriate team member.
  • Collaborates with patient/family when developing the plan of care, adjusting communication style as necessary.
  • Assists in assessment of family needs for bereavement services in collaboration with the IDT.
  • Completes all electronic documentation at the point-of-care, following the appropriate protocol.
  • Significant status changes in patient condition are identified, communicated, responded to, and documented promptly including, communication to the home care based IDT.
  • Reports patient/caregiver/employee incidents according to TRU policy.
  • Creates a supportive environment that respects each patient’s dignity, privacy, and cultural preferences.
  • Provides medication management per protocol. Recognizes and reports adverse drug reactions and medication variances per policy and procedure.
  • Demonstrates responsibility for scope of position/own standard of practice. Maintains required skills, licensure, regulatory requirements, and credentials needed to perform assigned duties.
  • Demonstrates effective verbal, written, and electronic communication skills; gives and receives feedback in a timely open manner; engages collaboratively in conflict resolution processes as needed
  • Provides leadership for evidence-based practice and collaborative or development of standards of care, programs of care, policies, procedures and protocols.
  • Provides leadership in maintaining a supportive, healthy work environment.
  • Function effectively within nursing and interprofessional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care and optimize clinical outcomes.
  • Is flexible and responsive in meeting and exceeding the expectations of all community partners, both internal and external
  • Participates in mandatory educational activities and required meetings
  • Is competent using agency information and technology systems
  • Is flexible, responsive, and adaptive in meeting organizational needs during peak times, this may include but is not limited to staffing, visits and assisting in other areas of the program.
