About The Position

The RN will be responsible for conducting initial health and ongoing clinical assessments, overseeing medication administration and adherence, and coordinating medical treatment with community providers. This role involves implementing client transfers to appropriate services, ensuring prescription medication procurement, maintaining records, providing staff training, and collaborating with directors and program staff for continuity of care. The position also requires providing emergency first aid/CPR assistance when needed.

Requirements

• NYS RN license
• Experience working with mentally ill and/or chemically dependent adults
• Ability to communicate effectively with residents and health care professionals.
• Flexibility and physical condition commensurate with the demands of the position.

Responsibilities

• Conducting initial health and ongoing clinical assessments.
• Overseeing administration of medications and monitoring adherence.
• Coordinating medical treatment and follow-up with community medical providers.
• Request and implement the transfer of medically or psychologically distressed clients to appropriate services, utilizing the established procedures.
• Ensure seamless procurement of prescription medication from partnering pharmacies.
• Maintaining records as per state and agency regulations.
• Providing training for program staff.
• Meeting with directors and program staff to ensure continuity of care.
• Providing emergency first aid/CPR assistance when needed.
