About The Position

Transitions Hospice is hiring an RN Case Manager to provide hospice care in Franklin County and surrounding areas. The role focuses on protecting patients’ dignity and comfort through physical, emotional, and spiritual support delivered where patients feel safest: at home. The RN serves as a clinical anchor for patients and families, coordinates care, guides the interdisciplinary team, and supports patients and caregivers through end-of-life care.

The schedule is Monday through Friday in eight-hour shifts, with on-call responsibilities. The listed pay range is $75,000–$85,000 an hour.

Requirements

• Active RN license in the state of employment.
• Graduation from an accredited school of Nursing.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication skills across care teams and families.

Nice To Haves

• One or more years of experience in hospice, home health, or a related organization is preferred.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand patients’ evolving needs.
• Build and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments according to physician orders.
• Maintain accurate, timely, and meaningful clinical documentation.
• Communicate with families, physicians, and community health partners.
• Educate patients and caregivers to help them feel confident and supported.
• Participate in the on-call rotation according to team policy.
