About The Position

Primary Care Solutions provides primary care services to Veterans through Community-Based Outpatient Clinics. This Registered Nurse position provides care as part of a Patient Aligned Care Team (PACT), with an active role in developing and implementing patient care plans. The role is full-time and onsite, with office hours Monday through Friday, 8:00 a.m. to 4:30 p.m.; remote work is not available. The nurse supports practitioners and patients, assesses and triages patient needs, coordinates care, and provides education and follow-up. The position also contributes to clinical goals and helps maintain a welcoming, professional atmosphere for Veterans and colleagues. It requires discretion and secure management of confidential and sensitive information.

Requirements

At least 1 year of Primary Care and/or Chronic Disease Management experience.
Current, active, and unrestricted state Nursing License.
Current BLS for providers from the American Heart Association, renewed annually.
Strong computer skills, including EMR experience.

Responsibilities

Assist practitioners with patient care and apply specialized nursing skills in patient assessment.
Triage urgent and emergent same-day patients, identify acute and high-acuity needs and Veterans at risk for uncoordinated care, and mobilize the care team to support optimal outcomes.
Help patients navigate and coordinate their care.
Participate in a daily team huddle.
Answer and return patient telephone calls, messages, and alerts.
Provide patient and family education focused on self-management, prevention, and wellness.
Monitor the discharge list and contact patients for follow-up.
Maintain a warm, welcoming, and professional atmosphere for colleagues and Veterans, ensuring their needs and expectations are consistently met.
Promote achievement of clinical goals and metrics.
Use discretion and securely manage confidential and sensitive information.
