This Registered Nurse position is within the Program Support Services division for Metro Atlanta and reports to the R.N., Medical Services Coordinator. The FLSA status is Exempt.

Responsibilities

  • Assist Medical Services Coordinator with new consumer assessments.
  • Perform follow up assessments as needed.
  • Visit hospitals for consumers as needed.
  • Attend discharges from the hospital after surgeries and other serious inpatient treatment.
  • Obtain all discharge information and communicate with direct care staff on necessary follow up information.
  • Communicate discharge information to Medical Services Coordinator and appropriate Directors.
  • Receive medication from the pharmacy and review with Program Manager or the CLS to assure accuracy of order.
  • Assist Medical Services Coordinator with supervision of self-administration of medication training and updates, as required by Policies & Procedures.
  • Take alternate call every other week with Medical Services Coordinator.
  • Use Medical Services Coordinator as back-up when taking calls, or as needed in emergency cases.
  • Train staff in using appropriate body mechanics to lift, move, position and provides prescribed exercises and physical care to resident.
  • Attend annual ISP meetings by special request from Associate Area Director.
  • Complete nursing activity notes on consumers, parents, and/or events, legibly, dated & signed in ink with no abbreviations.
  • Communicate with Medical Services Coordinator /Director Program Support/Associate Executive Director regarding medical concerns.
  • Assist RN in obtaining information for health risk screening tool & determining frequency of monitoring consumers’ health needs.
  • Routinely, on a monthly basis, assess those consumers in the day program identified by Medical Services Coordinator and Associate Executive Director as more fragile. Report findings to Medical Services Coordinator and Associate Executive Director.
  • Monitor as needed, via site visits those consumers identified as more fragile and/or in need of specific medical attention.
  • Monitor charts & sites and complete quality assurance as required.
  • Monitor homes and day program for continuity of care.
  • Comply with policies and procedures to ensure that confidentiality requirements are upheld.
  • Support and protect the fundamental human, civil, constitutional and statutory rights of clients and families as defined by agency.
  • Demonstrate competence in handling medical emergencies in accordance with UCP policies and procedures.
  • Adhere to all agency policies and procedures.
  • Develop and maintain positive and productive relationships with consumers, families, co-workers, and community members.
  • Maintain objectivity in position in order to set appropriate limits while working with consumers.
  • Attend all department meetings.
