About The Position
AdvisaCare is seeking Registered Nurses (RNs) to join their Skilled Nursing team. This role involves providing intermittent care to Medicare beneficiaries in their homes, often following an inpatient stay due to illness or surgery. The goal is to help patients regain independence and confidence in managing their health at home. The position emphasizes one-on-one care and building real relationships, offering a chance to make a meaningful difference in patients' lives.
Requirements
- Michigan RN License
- Michigan Driver's license and proof of insurance
- CPR
- Critical Thinking and Task Skills Necessary
- Computer Skills a Must
Nice To Haves
- 1 year Home Health Experience Preferred
- ER or Critical Care Experience a Plus
- OASIS Experience Preferred
Responsibilities
- Help patients understand and manage their disease process
- Dress wounds and provide skilled treatments
- Draw blood and manage drains, catheters, PICCs, and ostomies
- Evaluate home safety and patient needs
- Support medication and disease management
- Connect patients with helpful community resources
- Coordinate care with physicians, providers, caregivers, therapists, and families
