About The Position

This role is for a Registered Nurse in the Wellness Preventive Care department at Ascension Medical Group Clinic in Carmel, IN. The position is PRN, with shifts Monday-Friday during days only. The nurse must be available for early morning (starting at 6 am) and early evening (ending at 6 pm) shifts, depending on the coverage location. Coverage locations include Carmel Clay Schools Wellness Center, Zionsville Community Schools Wellness Center, and Heritage Wellness Clinic - Indianapolis.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required. Valid state license required specific to the state in which the associate works.
Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.

Responsibilities

Lead the clinical patient flow by preparing patients for examinations and assisting providers with outpatient procedures to ensure a seamless, high-quality visit.
Coordinate the continuum of care by managing referrals, follow-ups, and specialty communication to bridge the gap between the clinic and outside providers.
Support patient health literacy by translating complex physician orders into clear, actionable education plans that empower patients to manage their health at home.
Streamline treatment implementation by accurately executing clinical orders and documenting patient progress to maintain an airtight record of their long-term care journey.
