The Registered Nurse (RN) in this role will demonstrate performance consistent with professional standards of practice, the Nurse Practice Act, and the organization's mission and goals. This RN will manage patient care across the healthcare continuum to achieve optimal outcomes safely and cost-effectively, utilizing skills in collaboration, delegation, negotiation, and resource management. The primary focus will be on patients with wounds older than 30 days that are not healing properly. Responsibilities include direct patient care, performing intakes (initial assessments, charting), discharges (instructions, dressings), and functioning in a case management capacity. This position is located at the Community East Wound Care Clinic.

Requirements

  • Demonstration of performance consistent with professional standards of practice, care, the Nurse Practice Act and the mission and goals of Community Health Network.
  • Demonstration of skills in collaboration, delegation, negotiation and management of resources.
  • Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience.
  • Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC).

Responsibilities

  • Demonstrate performance consistent with professional standards of practice, care, the Nurse Practice Act, and the mission and goals of Community Health Network.
  • Manage patient care in the healthcare continuum to achieve optimum outcomes in a safe and cost-effective manner.
  • Demonstrate skills in collaboration, delegation, negotiation, and management of resources.
  • Care for patients with wounds that are greater than 30 days old and are not healing properly.
  • Act as a direct care professional with regard to wound care patients.
  • Perform intakes (initial assessments, charting).
  • Perform discharges (instructions, dressings).
  • Function in a case management role.
