About The Position

This role is for a Registered Nurse specializing in Wound Care at All Saints Hospital in Racine, WI. The position is full-time, working day shifts. The nurse will deliver compassionate, high-quality direct patient care adhering to best practices and protocols. They will manage the patient's comprehensive care journey, monitor and communicate critical status updates to the multidisciplinary team, and coordinate patient services across clinical disciplines. The role also involves executing physician and nursing orders accurately and promptly, and facilitating comprehensive discharge planning.

Requirements

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Nice To Haves

  • Certification specializing in Wound Care credentialed from the National Alliance of Wound Care and Ostomy (NAWCCB) preferred.

Responsibilities

  • Deliver compassionate, high-quality direct patient care that strictly adheres to established, patient-first best practices and protocols.
  • Manage the patient's comprehensive care journey, meticulously monitoring and communicating critical status updates to the multidisciplinary team to ensure continuity and safety of care.
  • Coordinate patient services across all clinical disciplines, acting as the primary liaison to guarantee seamless and well-integrated care for optimal patient outcomes.
  • Execute physician and nursing orders accurately and promptly to ensure the timely advancement of the patient's treatment plans.
  • Facilitate comprehensive discharge planning by proactively assessing patient needs and collaborating with the healthcare team to ensure a safe, smooth, and successful transition home.
