About The Position

Merakey is seeking a compassionate Visiting Registered Nurse (RN) Case Manager to join their team at their program in Warminster, PA. This role involves providing direct nursing care to individuals with developmental and physical disabilities. The responsibilities include clinical assessments, medication administration, and collaborating with a multidisciplinary team to ensure comprehensive care. The position offers an hourly rate of $43 with potential for enhanced pay, and travel to Bucks, Montgomery & Philadelphia Counties is involved, with an in-office component. The working hours are 9:00 am - 5:00 pm, Monday to Friday, with no weekends required.

Requirements

  • Graduation from an accredited Registered Nurse program.
  • Current license as an RN in the Commonwealth of PA.
  • Genuine interest in working with individuals with physical and developmental disabilities.
  • Possession of a current and valid PA driver's license, along with reliable transportation (may be required).
  • Ability to perform professional nursing duties including assessment, treatment, and medication dispensing.
  • Physical capability to exert up to 50lbs occasionally, and/or up to 20lbs frequently, and/or up to 10lbs continuously.

Responsibilities

  • Administer medications and treatments, including tube feedings and tracheostomy care, with precision and compassion.
  • Perform nursing assessments on residents as needed to identify and address health issues promptly.
  • Collaborate with various healthcare professionals and specialists to coordinate comprehensive care, keeping the nurse manager informed.
  • Provide health education to residents and their families, including implementing self-medication programs as directed by the nurse manager.
  • Conduct in-service training for staff to ensure compliance with protocols and procedures, promoting a culture of excellence in care delivery.
  • Communicate health issues with the interdisciplinary team, actively participating in staff meetings and monthly reviews with the QIDP.
  • Complete quarterly assessments in a timely manner, maintaining accurate documentation.
  • Fulfill duties as a charge nurse when assigned, carrying an emergency contact telephone, and responding as required.
  • Perform other duties as assigned, contributing to the smooth operation of our program.
