About The Position

Tryfacta is seeking a Registered Nurse (RN) for our client in Rockford IL 61108. This is a 13-weeks contract assignment. The role involves coordinating care with various healthcare professionals, scheduling appointments, monitoring client health, training support staff, delegating nursing tasks, maintaining medical records, and participating in an on-call rotation.

Requirements

  • Active and unencumbered Registered Nurse (RN) license.
  • Minimum one year of clinical registered nursing experience within the last five years.
  • Strong clinical assessment and patient-care skills.
  • Excellent care coordination and follow-up skills.
  • Strong communication and interpersonal skills.
  • Ability to train, support, and delegate nursing tasks to care staff.
  • Strong medical documentation and record-management skills.
  • Detail-oriented with strong organizational and time-management abilities.
  • Ability to work collaboratively with healthcare providers and multidisciplinary teams.
  • Ability to respond effectively to health-related emergencies and on-call situations.
  • Ability to stand, walk, bend, reach, kneel, climb, stoop, crouch, and balance as required.
  • Ability to lift up to 25 lbs. independently and push/pull up to 40 lbs. of force.
  • Ability to safely assist with patient/client transfers.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred.

Responsibilities

  • Coordinate care with healthcare providers, nutrition services, and behavioral health specialists.
  • Schedule and coordinate medical appointments and ensure timely follow-ups.
  • Monitor clients' health needs and provide appropriate care coordination.
  • Train and support Direct Support Team members in medication administration and nursing tasks.
  • Delegate nursing responsibilities in accordance with state regulations and Mosaic policies.
  • Maintain accurate and up-to-date medical records.
  • Review and audit medical documentation for accuracy and completeness.
  • Monitor health concerns and ensure proper documentation and follow-up.
  • Communicate with clients, staff, healthcare providers, and supervisors to ensure continuity of care.
  • Participate in an on-call rotation approximately every third week, primarily providing phone support for emergencies.
