About The Position
Human Touch Home Health is seeking an experienced and compassionate Registered Nurse (RN) to join our growing team serving patients throughout Fairfax, Prince William and Loudoun Countites. This PRN opportunity offers flexibility, professional independence, and the chance to build meaningful relationships while delivering exceptional patient care. Home health nursing is about more than providing skilled care; it's about restoring confidence, supporting recovery, preventing hospitalizations, and helping patients remain at home. As a Home Health Registered Nurse, you'll provide personalized, one-on-one care that helps patients recover from illness, manage chronic conditions, and maintain their independence. You'll build lasting relationships, educate patients and caregivers, and witness the progress your care makes every day. This role offers meaningful nursing, stronger patient connections, and better outcomes, without the constraints of crowded hospital units or rushed patient assignments.
Requirements
- Active Virginia Registered Nurse (RN) License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment, Critical Thinking, and Documentation Skills
Nice To Haves
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
Responsibilities
- Perform comprehensive nursing assessments and skilled visits
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
- Develop and implement individualized plans of care
- Provide skilled nursing services, including medication management, wound care, disease management, and patient education
- Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
- Coordinate care with therapists, social workers, and home health aides
- Complete timely and accurate clinical documentation using WellSky
- Educate patients and caregivers to promote safety, independence, and improved health outcomes
