About The Position

Be the Nurse Who Redefines Care. Step outside the traditional hospital walls and deliver hospital-level care directly in patients’ homes. This innovative role blends the autonomy of delivering care in the home setting with the complexity of acute inpatient nursing, supported by a collaborative interdisciplinary team.

Requirements

Active RN license in the state of Illinois
BLS certification (AHA)
Valid driver’s license with a clean driving record
2+ years of recent acute inpatient care or ED experience preferred
Strong IV, phlebotomy, and clinical assessment skills
Ability to work independently in home settings
Excellent communication and critical thinking
Comfortable with technology and remote monitoring tools

Responsibilities

Deliver acute, hospital-level care in patients’ homes
Perform skilled clinical tasks (IV starts, phlebotomy, medication administration)
Monitor patient conditions and recognize changes requiring escalation
Collaborate with a virtual and in-person interdisciplinary team
Use evidence-based practice to ensure safe, patient-centered care
Coordinate discharge planning and ongoing care needs
Promote a culture of safety and high-quality outcomes
