About The Position

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. Every weekend or every other weekend working 8am-8pm covering all things On-Call!

Requirements

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
• Ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to work closely with patients on a 1-1 level.
• Ability to work independently and on your own from a day-to-day basis.
• Proficiency in phone, email, and written communication.
• Experience in meetings with a variety of healthcare professionals.
• Understanding of hospice rules and regulations.
• Strong time management skills to manage visits, documentation, and meetings.
• Ability to prioritize and triage multiple concerns.
• Ability to learn an electronic medical records system.
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area.

Responsibilities

• Covering On-Call duties every weekend or every other weekend from 8am-8pm.
