About The Position

Responsibilities include measuring vital signs, administering IVs, changing dressings, cleaning wounds, providing skilled nursing care, preventative rehabilitative procedures, and prescribed treatments to diverse patient populations in various home settings. The RN will collaborate closely with a supportive patient care team, including physicians, therapy, social services, pastoral care, and home health aides. Additionally, the role requires providing patient and family education for disease management and health promotion as directed by the physician's plan of treatment, all while maintaining the highest standards of care.

Requirements

  • New York State RN license
  • Minimum of one year of recent hands on experience as a Registered Nurse.
  • Basic Life Support Certification
  • Valid New York State Driver’s License and reliable transportation

Responsibilities

  • Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
  • Measure vital signs, Administer IVs, change dressings, clean wounds as needed
  • Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
  • Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
  • Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
