About The Position

Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual/disabilities and homelessness to help stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice. We are seeking qualified Registered Nurse (RN ) interested in joining our ACT Case Management Hope team at our Claymont, Delaware location.

Requirements

  • Board Certified Licensed RN in the state of Delaware; or participating in the Nursing Licensure Compact to include the state of DE.
  • Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for participant rights and personal preferences in treatment are essential.
  • Knowledge of mental health and substance abuse systems and supports.
  • Good communication skills both oral and written.
  • Ability to work independently and as part of a focused team.
  • Strong organizational and analytical skills.
  • Possess a valid driver’s license with an acceptable driving record.

Nice To Haves

  • MA/BA in nursing or a registered nurse with a Mental Health Certification from the American Nursing Association, preferred.

Responsibilities

  • Coordinate the behavioral and physical health care needs of the program participants.
  • Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participants and assigned staff.
  • Maintains a caseload of assigned ACT/ICM participants and completes assigned paperwork in a timely manner.
  • Provides ongoing assessment of participant’s mental health symptoms and responds to recovery plans.
  • Educates and supervises participants in self-administration of medication.
  • Ensures annual review of the participants physical condition and assist access to and encourage compliance with treatment for identified physical problems.
  • Act as a liaison consulting with community agencies and families to maintain coordination of care in the treatment process.
  • Monitors medication, documenting participant compliance, medication efficacy, and side effects as reported.
