About The Position

A Community Admissions Registered Nurse is responsible for the presentation of Hospice services to clients who have been referred for all levels of care (general inpatient, home care, and nursing home). This role facilitates and completes the admission process after reviewing patient/family goals of care and identifying the appropriate level of care. The RN initiates the individual professional IDT plan of care and collaborates with the IDT, physician, and facility staff to determine start of care orders. Additionally, the RN provides in-services for hospital staff and the community.

Requirements

  • Current Registered Nurse licensure in NY
  • Valid Driver's License, free of serious violations
  • Vehicle required for use when on duty or on-call

Nice To Haves

  • Certification in Hospice and Palliative Care Nursing
  • 2 years of nursing practice as a professional Registered Nurse

Responsibilities

  • Present hospice services to patients and families in an individualized, sensitive manner, demonstrating knowledge of hospice eligibility guidelines and HPCB programs.
  • Perform assessments to identify physical, psychosocial, environmental, safety, emotional, and spiritual needs related to terminal illness and document findings in the EMR.
  • Perform pain assessments, adhere to pain protocols, and utilize equi-analgesic dosing for optimal pain relief with timely interventions.
  • Articulate the clinical course/disease progression of various chronic illnesses.
  • Articulate the etiologies of symptom manifestation related to a disease process.
  • Initiate the plan of care timely and appropriately, anticipating potential problems.
  • Demonstrate knowledge of common medications, including usage, indications, adverse side effects, contraindications, and interactions.
  • Organize work to meet productivity standards, incorporating computer documentation into patient visits and completing documentation within time parameters.
  • Execute patient/family/caregiver teaching in all situations.
  • Establish and maintain positive rapport with other access and hospice clinical staff, hospice support staff, hospital staff, and the medical community.
  • Participate in the weekend and holiday schedule.
