About The Position

Suncrest At Brighton/Suncrest Hospice aims to redefine hospice care by offering exceptional service and care to patients. This is achieved by ensuring adequate staff-to-patient ratios, which allows for increased clinical visits and reduced clinician caseloads. Requirements

  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
  • Ability to collaborate effectively within a team environment with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Ability to work independently on a day-to-day basis.
  • Understanding of hospice rules and regulations.
  • Time management skills to manage visits, documentation, and meetings.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver's license, and auto insurance to travel across the service area.

  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Work closely with patients on a 1-1 level but get strong support from a patient care team.
  • Activities include: phone, email and written communication, meetings with a variety of healthcare professionals.

Responsibilities

  • Provide care and services for a group of patients.
  • Visit patients in their residences, including private homes, Skilled Nursing Facilities, and assisted living facilities.
  • Serve as a contributing member of the interdisciplinary team.
  • Provide expertise in psych-social assessments, interventions, and knowledge of community resources.
  • Work as a Resource.
