About The Position

Comfort Home Care is seeking a Registered Nurse (RN) for a full-time opening on the morning shift (Monday through Friday) in the Haverhill area. Requirements

Active Registered Nurse (RN) license in Massachusetts.
One year of nursing experience (home health or psych preferred).
A drive to provide compassionate, patient-centered care.
Reliable transportation for patient visits.

Responsibilities

Assess and Adapt: Evaluate patient's current condition, medical needs, and overall well-being during each visit.
Create and Modify Care Plans: Actively shape and adjust patients' treatment plans based on their needs and changing conditions.
Teach and Support: Provide medication guidance to psychiatric patients and wound care education to post-op patients to help them stay independent at home.
Communicate and Coordinate: Keep providers, families, and care teams informed of any changes to ensure patients receive timely and appropriate care.
Document and Reflect: Submit visit notes accurately from home for compliance and continuity of care.
