About The Position

Emplify Health by Gundersen is seeking a part-time RN (0.6 FTE) to perform Medicare Annual Wellness Visits. This is an opportunity to work in an autonomous environment to the full scope of your license in a team-based-care model. The role involves performing Medicare Annual Wellness Visits, including referrals, consults, immunizations, and setting up screenings. The position offers daytime hours, no weekends, no holidays, and no on-call duties, with 8-hour shifts between 7:30 am and 5:30 pm. The role is hybrid/remote, allowing up to 50% work from home based on department needs, with onsite locations in La Crosse and Onalaska.

Requirements

• Associate Degree in Nursing
• RN licensure to practice in the state of Wisconsin upon hire
• RN licensure to practice in the state of Minnesota & Iowa within 3 months of hire
