About The Position

Connections Health Solutions is seeking a Registered Nurse (RN) for a part-time position at the RESET Center in San Francisco. The RESET Center provides an alternative destination for individuals found to be publicly intoxicated, diverting them from the emergency department or jail. This role involves providing immediate-access behavioral health crisis care, stabilizing individuals, and connecting them to community resources. The RN will perform nursing assessments, monitoring, and protocol-driven interventions in a low-barrier, sobering environment, utilizing harm-reduction and trauma-informed best practices. The position ensures safe oversight, supportive care, and appropriate referrals or transfers for individuals in acute intoxication.

Requirements

Graduation from an accredited nursing program
Unrestricted, active license as a Registered Professional Nurse through the California State Board of Nursing
Basic computer literacy, including the ability to document in an electronic health record (EHR)
Ability to obtain and accurately record vital signs
Strong attention to detail and ability to complete documentation in a timely manner
Ability to successfully complete Basic Life Support (BLS) certification during the first week of employment
Must obtain Overdose Prevention and Narcan Administration training within 30 days of hire
Fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company’s exemption process prior to their start date as a condition of employment

Nice To Haves

Three (3) years of experience in psychiatric, behavioral health, substance use treatment, detox, or related settings
Previous experience in emergency room, crisis stabilization, or alternate destination models
Bachelor's degree in nursing (BSN)
Current Basic Life Support (BLS) certification
Experience working with individuals with addiction, medication-assisted treatment, and harm-reduction practices

Responsibilities

Provides nursing assessment, monitoring, and protocol-driven intervention for adults experiencing acute intoxication within a low-barrier sobering environment.
Performs abbreviated nursing triage per standardized procedure, assessing behavioral and physical health needs.
Documents in accordance with harm-reduction and trauma-informed best practices.
Ensures safe oversight, supportive care, and protocol-guided referrals or transfers for individuals who may otherwise be transported to an emergency hospital or jail.
Performs abbreviated nursing assessments, including vitals, impairment/intoxication levels, and withdrawal risk screening.
Recognizes signs of medical or psychiatric emergencies and initiates EMS or ED transfers when clinically indicated.
Provides supportive care, including hydration, nutrition/snack support, wound care, and medication administration per standing orders for minor injuries or symptoms.
Assesses and monitors patients through the sobering process, noting changes in behavior, cognition, or level of consciousness.
Offers education, motivational engagement, and compassionate communication regarding substance use, health risks, and harm-reduction strategies.
Utilizes trauma-informed and de-escalation approaches to respond to escalating behaviors.
Conducts incident debriefs and completes required clinical documentation following escalations or emergency outcomes.
Collaborates, as appropriate, with EMS, law enforcement, and emergency department teams as part of alternate destination protocols.
Supports center-wide safety and risk mitigation practices across the multidisciplinary team.
Participates in required onboarding and ongoing clinical training; maintains competencies and attends required meetings.
Performs all other duties as assigned within the scope of practice.
