Suncrest At Brighton/Suncrest Hospice is a CHAP-certified hospice provider that is committed to changing the expectation of hospice care. They are looking for individuals with a commitment to providing the highest quality of care to patients and their families. The Registered Nurse (RN) will provide compassionate, individualized hospice care wherever patients call home, including private residences, skilled nursing facilities, and assisted living communities. The RN will collaborate with an interdisciplinary team to develop care plans that support the physical, emotional, and spiritual needs of patients and their families. The company prioritizes quality over quantity by maintaining lower clinician caseloads and higher staff-to-patient ratios, allowing nurses to spend more meaningful time with patients and families.
Requirements
- Current Registered Nurse (RN) license in good standing in the state of practice
- A passion for providing exceptional, patient-centered hospice care
- Strong collaboration and communication skills with interdisciplinary care teams
- Ability to work independently while effectively managing your daily schedule
- Excellent time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs
- Comfort learning and utilizing electronic medical record (EMR) systems
- Reliable transportation, a valid driver's license, and current auto insurance to travel throughout the service area
Responsibilities
- Provide compassionate, individualized hospice care wherever patients call home—including private residences, skilled nursing facilities, and assisted living communities.
- Collaborate with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans that support the physical, emotional, and spiritual needs of patients and their families.
