About The Position

The Registered Nurse (RN) will uphold principles of dignity, respect, and person-centered care. This role involves providing clinical oversight to LPN/LVNs and non-nursing staff, developing and implementing individualized care plans, and conducting nursing assessments. The RN will administer medications and treatments, monitor individuals for changes in health status, and delegate nursing tasks appropriately. Responsibilities also include ensuring proper medication management, providing health-related training, participating in team meetings, coordinating with healthcare providers, and responding to emergencies. Maintaining accurate documentation, supporting compliance with regulations, and participating in quality assurance are key aspects of this position. The role also includes providing telephone triage nursing support during on-call shifts.

Requirements

  • Associate's degree in nursing (ASN) or Bachelor’s degree in nursing (BSN) from an accredited nursing program.
  • Minimum of 1 year of clinical nursing experience required.
  • Active RN license from state of residence. If work location is in a different state, must hold a multistate RN license in accordance with the rules for Nurse Licensure Compact (NLC).
  • Valid driver’s license, with proof of auto insurance coverage.
  • Basic Life Support (BLS) certification.
  • Understanding of developmental and intellectual disabilities, health disparities, and person-centered care models.
  • Strong clinical assessment, critical thinking, and documentation skills.
  • Excellent interpersonal communication and team collaboration abilities.
  • Ability to communicate effectively with clients, families, interdisciplinary team members, and supervisors, verbally and in writing.
  • Ability to manage competing priorities and respond to emergencies effectively.
  • Proficiency in electronic health record (EHR) programs and Microsoft 365 programs including Outlook, Teams, and Word.
  • Visual acuity and hearing must be sufficient for assessments, medication verification, and responding to alarms or client needs.
  • The RN must be able to respond quickly to emergencies, including physical intervention if needed for safety.

Nice To Haves

  • Experience with IDD/DD populations preferred.
  • Familiarity with state and federal regulations governing IDD services (e.g., CMS ICF/IID, HCBS Waivers) is preferred.

Responsibilities

  • Uphold principles of dignity, respect, and person-centered care in all interactions with individuals.
  • Provide clinical oversight to LPN/LVNs and other non-nursing staff as directed.
  • Develop and implement individualized care plans in collaboration with interdisciplinary team members.
  • Conduct initial and ongoing nursing assessments.
  • Administer medications and treatments per physician/Advanced Practice Provider (APP) orders and applicable regulations.
  • Monitor and evaluate individuals for changes in health status and intervene accordingly.
  • Delegate and oversee nursing tasks to unlicensed staff and other approved caregivers only when appropriate to scope, and only to those who have been properly trained with demonstrated competency.
  • Ensure proper storage, handling, preparation, and labeling of medications.
  • Maintain accurate medication inventory and immediately report any discrepancies.
  • Provide health-related training to direct support staff, individuals, and caregivers.
  • Participate in interdisciplinary team meetings, care reviews, and service planning.
  • Coordinate with healthcare providers, pharmacies, and other service agencies to ensure continuity of care.
  • Respond immediately to emergencies and provide nursing assistance.
  • Ensure a safe environment for individuals, staff, and others and immediately report safety concerns.
  • Maintain accurate, timely, and confidential clinical documentation in compliance with Caregiver policies and HIPAA requirements.
  • Submit accurate and timely billing documentation, when applicable.
  • Support compliance with licensing and regulatory requirements (e.g., CMS, HHS IDD, etc.).
  • Participate in quality assurance and improvement activities.
  • Provide telephone triage nursing support for assigned on-call shifts, including after-hours as needed.
