About The Position

FHS is accepting applications for the final hiring phase of our 20-bed, skilled nursing facility addition. We expect to hire for Day, Evening and Night positions at the end of April into May 2026. Nurses play a vital role in the care of our residents. Farmington Health Services (FHS) is a 90-bed community consisting of; Trinity Care Center: LTC and TCU, The Terrace: Assisted and Independent Living and The Suites: Memory Care. Requirements

Strong clinical assessment skills
Proficiency in using electronic health record systems
The ability to make sound judgments in emergency situations
Knowledge of various non-invasive medical treatments

Responsibilities

Resident Assessment and Care Planning
Collaborate with other healthcare team members
Medication administration
Monitoring activities
Providing emotional support for our Residents well being
Maintaining confidentially and HIPAA
Oversee our CNAs and TMAs
Assist with Direct Care/ Stand-By when necessary
Accurate and thorough documentation
Insure the safety and wellness of our Residents and Staff
