About The Position
Guardian Angel Senior Services is hiring an experienced Registered Nurse (RN) to join our home care team serving Boston and the Greater Boston area. This is a flexible nursing position that offers independence, meaningful one-on-one client interactions, and the opportunity to make a lasting difference. Requirements
- Current Massachusetts Registered Nurse (RN) license
- Ability to work independently
- Reliable transportation, valid driver's license, and automobile insurance
Nice To Haves
- Home care, community health, case management, or visiting nurse experience preferred
- Strong assessment and clinical judgment skills
- Excellent communication and documentation skills
Responsibilities
- Complete comprehensive in-home client assessments
- Develop, review, and update individualized care plans
- Conduct caregiver orientation and quarterly supervisory visits
- Educate, coach, and support caregivers to ensure quality care
- Complete documentation within required timeframes
- Monitor client progress and communicate changes to the care team
- Collaborate with office staff to ensure continuity of care
- Participate in caregiver education and in-service training
- Perform routine nursing visits as needed, including medication management, blood pressure checks, catheter changes, and other nursing services within your scope of practice
- Provide hands-on assistance during urgent situations when needed
- Complete in-home assessments and develop Home Health Aide Plans of Care (for ASAP clients)
- Match caregiver skills with client care needs (for ASAP clients)
- Orient and supervise caregivers assigned to ASAP clients
- Complete quarterly supervisory visits and annual reassessments (for ASAP clients)
- Collaborate with ASAP nursing staff regarding client status and care plan updates
- Identify changes in client condition and advocate for appropriate services (for ASAP clients)
- Report required incidents and changes according to ASAP guidelines (for ASAP clients)
