About The Position
Human Touch Home Health is seeking an experienced and compassionate Registered Nurse (RN) to join our growing team providing skilled home health services throughout Washington, DC. This is a PRN opportunity that offers flexibility, professional autonomy, and the chance to build meaningful relationships while delivering exceptional patient care. Home health nursing focuses on restoring confidence, supporting recovery, and preventing hospitalizations by helping patients remain in their homes. As a Home Health Registered Nurse, you will provide skilled, individualized care, develop meaningful relationships with patients and their families, and educate, encourage, and advocate for your patients while seeing the direct impact of your care daily. This role offers personalized nursing that puts the patient first, without the constraints of crowded hospital units or rushed assignments.
Requirements
- Active Washington, DC Registered Nurse (RN) License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment, Critical Thinking, and Documentation Skills
Nice To Haves
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
Responsibilities
- Perform comprehensive nursing assessments and skilled visits
- Develop, implement, and update individualized plans of care
- Provide skilled nursing services, including medication management, wound care, disease management, and patient education
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments as appropriate
- Monitor patient progress and communicate changes to physicians and the interdisciplinary care team
- Coordinate care with therapists, social workers, and home health aides
- Complete timely and accurate clinical documentation using WellSky
- Educate patients and caregivers to promote safety, independence, and improved health outcomes
