About The Position

James River Hospice is actively seeking a compassionate and highly committed Hospice RN Case Manager to join their growing team. The role involves the hospice case management of assigned patients on an intermittent basis in their place of residence and coordinating with the interdisciplinary team to ensure an optimal care experience for the patient.

Requirements

Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia.
At least one year of hospice experience is required.
Graduate of an accredited nursing school.
Must have a valid driver's license and a reliable car available for use during work hours.

Responsibilities

Completing an initial assessment of the patient and family to determine hospice needs.
Developing an individualized plan of care focused on comfort and quality of life.
Managing and coordinating care with the interdisciplinary team.
Supervising LPNs and hospice aides.
Providing hands-on support for pain and symptom management.
Spending meaningful time with patients and families.
Monitoring, documenting, and communicating changes in condition.
Obtaining and carrying out physician orders.
Adjusting the plan of care as needed.
Participating in on-call rotation per policy.
Performing other duties as assigned.
