About The Position

Clarvida is seeking a Registered Nurse (RN) to join their Assertive Community Treatment (ACT) team. The RN will advocate for and work with assigned adult clients, overseeing their medical needs. This includes conducting nursing assessments, reviewing health histories, monitoring medication responses, identifying the need for physician referrals, and assisting with Patient Assistance Program applications. The role involves monitoring clients, consulting with them and their family supports on medical, nutritional, and other health issues related to mental health or substance use disorders, and educating clients on self-administration of medication. A significant part of this role involves meeting with clients in the local community.

Requirements

Registered Nurse (RN) License
Valid Georgia Driver’s License and clean Motor Vehicle Record (MVR)
Clean Criminal Back Ground Check (no conviction or credible evidence of abuse, neglect, or sexual exploitation-child or adult)

Responsibilities

Oversee the medical needs of clients.
Conduct nursing assessments and review health histories.
Monitor client response to medications.
Identify the need for physician referrals.
Assist clients with applying for Patient Assistance Program medications.
Consult with clients and their family supports regarding medical, nutritional, and other health issues related to mental health or substance use disorders.
Provide education on self-administration of medication.
Meet with clients in the local community.
