About The Position

Human Touch Home Health is seeking an experienced and compassionate Registered Nurse (RN) to join our growing team serving patients throughout Blacksburg, Christiansburg, Radford, Pulaski, Floyd, Giles County, Montgomery County, and surrounding New River Valley communities. This PRN opportunity offers flexibility, professional independence, and the chance to make a lasting impact on the lives of patients. Home health nursing involves building trust, supporting recovery, educating families, and creating confidence. As a Home Health Registered Nurse, you will provide skilled, individualized care in patients' homes to help them recover from illness, manage chronic conditions, prevent hospitalizations, and maintain independence. This role allows for meaningful one-on-one patient care and the opportunity to change lives daily.

Requirements

Active Virginia Registered Nurse (RN) License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment and Critical Thinking Skills
Excellent Communication and Documentation Skills

Nice To Haves

Home Health Experience Preferred but Not Required
OASIS experience is a plus

Responsibilities

Perform comprehensive patient assessments
Develop and implement individualized plans of care
Provide skilled nursing services, medication management, wound care, and patient education
Monitor patient progress and communicate changes to physicians and the interdisciplinary care team
Coordinate care with therapists, social workers, home health aides, and other healthcare professionals
Complete timely and accurate clinical documentation
Educate patients and caregivers to promote health, safety, and independence
