About The Position
Human Touch Home Health is seeking an experienced and compassionate Registered Nurse (RN) to join our growing team serving patients throughout the rural communities surrounding Fredericksburg, Virginia, including King George, Caroline County, Orange County, Locust Grove, Bowling Green, Colonial Beach, King William, King and Queen, and surrounding areas. This PRN opportunity offers flexibility, professional independence, and the chance to make a meaningful impact in underserved communities. As a Home Health Registered Nurse, you will provide skilled, individualized care to patients in rural communities, helping them recover from illness, manage chronic conditions, prevent hospitalizations, and remain safely in their homes. Your care bridges the gap for patients who depend on experienced clinicians to bring quality healthcare directly to their doorstep.
Requirements
- Active Virginia Registered Nurse (RN) License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment, Critical Thinking, and Documentation Skills
Nice To Haves
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
Responsibilities
- Perform comprehensive nursing assessments and skilled visits
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
- Develop and implement individualized plans of care
- Provide skilled nursing services, including medication management, wound care, disease management, and patient education
- Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
- Coordinate care with therapists, social workers, and home health aides
- Complete timely and accurate clinical documentation using WellSky
- Educate patients and caregivers to promote safety, independence, and improved health outcomes
