PRN Registered Nurse (RN)
About The Position
Community Care Center is an inner-city Skilled Nursing Facility (SNF) serving a diverse population of geriatrics (long and short term) and individuals with behavioral health needs. This PRN (Per Required Need), on-call position offers flexibility with shifts, with a particular need for PM and Overnight shifts. The center is managed by Transitional Care Management, which provides comprehensive management services to various inpatient healthcare centers in Northern IL & the Chicagoland area, including Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
Requirements
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in IL.
- Prior floor nurse experience within a skilled nursing facility is strongly preferred.
- Experience with post-acute patients, specifically cardiac, post-op ortho, and medically complex.
- Electronic medical records experience is helpful (Point Click Care or PCC is ideal).
- Commitment to at least two (2) weekend shifts per month (Saturday and/or Sunday).
- Intermittent weekday availability as needed.
- Flexibility to work Day, PM, and NOC shifts (PMs and NOCs are most needed).
- Availability to work some holidays.
- Availability to attend a Wednesday New Employee Orientation.
Nice To Haves
- Experience with the post-acute patients – specifically the cardiac, post op ortho & the medically complex
- Electronic medical records experience is helpful (Point Click Care or PCC is ideal)
Responsibilities
- Provide nursing care as a floor nurse.
- Attend New Employee Orientation classes.

- Be available for shifts scheduled in advance or with short notice.
