Requirements

Active Registered Nurse (RN) license in Massachusetts.
One year of nursing experience (home health or psych preferred).
A drive to provide compassionate, patient-centered care.
Reliable transportation for patient visits.

Responsibilities

Assess and Adapt: Evaluate each patient's current condition, medical needs, and overall well-being during every visit.
Create and Modify Care Plans: Actively shape patients' treatment plans based on their needs and adjust care as conditions change.
Teach and Support: Provide guidance to psychiatric patients on medication and assist post-operative patients with wound care, helping them stay independent at home.
Communicate and Coordinate: Keep providers, families, and care teams informed of any changes to ensure patients receive the right care at the right time.
Document and Reflect: Submit visit notes accurately from home for compliance and continuity of care.
