Requirements

  • RN currently licensed in the State of Idaho
  • Drivers License and auto insurance

Responsibilities

  • Receives case referrals and reviews patient information to determine home health needs.
  • Assigns appropriate home health and hospice personnel to cases.
  • Conferences with Medical Director regarding patient eligibility for services.
  • Reviews and evaluates each case through home visits, conferences, record review, and clinician services.
  • Instructs and guides clinicians to improve performance and delivery of quality home care.
  • Assists clinicians in establishing therapeutic goals, setting priorities, and developing a plan of care.
  • Attends case conference meetings with hospice personnel for care coordination.
  • Conducts quarterly record reviews and communicates findings to the Director of Nursing and hospice personnel.
  • Assists in the screening and interviewing process of new employees and makes recommendations.
  • Assists in the orientation of new employees.
  • Assists Director of Nursing and supervisory hospice personnel in planning, implementing, and evaluating in-service and continuing education programs.
  • Assists in formulating, revising, implementing, and evaluating organization policies, procedures, goals, and objectives.
  • Complies with accepted professional standards and principles.
  • Assesses community health needs and assists in developing plans/programs to meet them.
  • Participates in public relations and community activities to promote the organization's role.
  • Promotes a customer service orientation to all hospice personnel.
  • Adheres to the organization's documentation and care procedures and standards of personal and professional conduct.
  • Participates actively in quality assessment performance improvement programs.
