Requirements
- RN currently licensed in the State of Idaho
- Drivers License and auto insurance

Responsibilities
- Receives case referrals and reviews patient information to determine home health needs.
- Assigns appropriate home health and hospice personnel to cases.
- Conferences with Medical Director regarding patient eligibility for services.
- Reviews and evaluates each case through home visits, conferences, record review, and clinician services.
- Instructs and guides clinicians to improve performance and delivery of quality home care.
- Assists clinicians in establishing therapeutic goals, setting priorities, and developing a plan of care.
- Attends case conference meetings with hospice personnel for care coordination.
- Conducts quarterly record reviews and communicates findings to the Director of Nursing and hospice personnel.
- Assists in the screening and interviewing process of new employees and makes recommendations.
- Assists in the orientation of new employees.
- Assists Director of Nursing and supervisory hospice personnel in planning, implementing, and evaluating in-service and continuing education programs.
- Assists in formulating, revising, implementing, and evaluating organization policies, procedures, goals, and objectives.
- Complies with accepted professional standards and principles.
- Assesses community health needs and assists in developing plans/programs to meet them.
- Participates in public relations and community activities to promote the organization's role.
- Promotes a customer service orientation to all hospice personnel.
- Adheres to the organization's documentation and care procedures and standards of personal and professional conduct.
- Participates actively in quality assessment performance improvement programs.
