About The Position

The Georgia Heart Institute Triage Nurses duties include but are not limited to: use of telephone etiquette to provide immediate assistance to the caller to assess, coordinate and maintain proper function of patient care in a high call volume environment for General Cardiology, Interventional Cardiology, and Electrophysiology across all locations in a remote setting. Utilize critical thinking and clinical decision-making skills along with approved nursing protocols to determine appropriate level of care needed based on assessment of patient symptoms. Proficiently document interactions in EMR to communicate and collaborate with all departments within GHI. Work independently on weekends and holidays to assist callers after normal business hours. Serve as liaison to relay critical information received to physician and care team.

Requirements

  • Professional Nurse with Current State License.
  • Current BLS.
  • Associates Degree.
  • Job Specific and Unique Knowledge, Skills and Abilities Demonstrates knowledge appropriate to care for adult and geriatric cardiac patient populations
  • Strong organizational and interpersonal skills
  • Excellent written and verbal communication skills
  • Ability to work effectively and independently in a remote setting
  • Ability to prioritize and multitask in a dynamic work environment
  • Strong analytical and problem-solving skills to make sound clinical judgements
  • Excellent understanding of medical terminology and the disease process
  • Advanced computer skills necessary to operate multiple computer systems in unison to provide the most efficient care (i.e. EPIC, Avaya, Microsoft, Outlook)

Nice To Haves

  • ACLS preferred
  • Three (3) years experience as Registered Nurse.
  • One (1) year Cardiology experience, critical care or ED.

Responsibilities

  • Triage accordingly. Answer calls promptly to assess patient symptoms and conditions to determine level of care by utilizing approved protocols.
  • Activate EMS, direct them to proceed to the ED, schedule a problem visit, or communicate with physician to further accommodate patient needs.
  • Document in EMR. Compile accurate data into EPIC systems. Update and verify information at time call (i.e. enter medications, demographics, or other necessary information in appropriate area of chart). Ensure accuracy of data and relay to clinical team in a proficient manner.
  • Provide excellent customer service. Greets each caller with a positive attitude, compassion and concern. Listens to each caller thoroughly and asks questions as appropriate.
  • Schedule Problem Visits. Use of clinical judgement and approved Triage Protocols based on information provided by the caller to determine if an office visit is appropriate.
  • Event Monitors. Responsible for receiving urgent event monitors reports and assessing patient symptoms at time of event. Communicating report findings and patient symptoms to the appropriate care team in real time.
  • Critical Results. Receive critical labs or urgent cardiac testing results and immediately relay to appropriate care team.
  • Patient Education. Relays approved information for the current treatment plan (i.e. medications, complex medical diagnoses, cardiac testing and procedures, results, event/Holter monitors, diet and nutrition). Review of documentation to answer patient questions regarding treatment plan.
  • Voicemails and In Baskets. Accountable for retrieving and addressing patient needs obtained from Avaya Message System and EPIC GHI Triage Nurse In Basket.
  • Physician Orders. Utilize physician notes to enter orders relevant to plan of care.
  • Medication Refills. Use the EPIC refill protocol to ensure all parameters are met to fulfill the medication request as appropriate.
  • Completes all duties as assigned. Ensures tasks are completed by end of day.
  • Facilitate patient care. Precise and thorough collection of pertinent data from Encounters to answer clinical questions from patients or other healthcare providers.
  • Resource for Mas, CMS, and LPNs. Based on knowledge base of education, all RNs are utilized as resources for clinical information.
  • Maintains awareness of any updates or changes to current policies and procedures affiliated with NGHS/GHI.
  • Performs other related job duties or functions as requested or assigned according to the needs of the organization.
