About The Position

As we continue to grow, we are seeking a dependable and compassionate PRN Registered Nurse (RN) to join our team and assist with in-home assessments and reassessments. This is a rewarding opportunity to make a meaningful impact while working with a supportive and mission-driven organization. This flexible PRN role is ideal for an RN looking for supplemental hours while making a meaningful impact in the community. The nurse will complete in-home visits, client assessments, and reassessments while collaborating with our care team to ensure exceptional client support and continuity of care.

Requirements

• Active RN license required
• Tech-savvy and comfortable using digital documentation systems
• Self-motivated with a flexible, “go-getter” attitude
• Reliable transportation for local visits

Nice To Haves

• Previous home health, hospice, or healthcare experience preferred
• Experience working with Medicaid clients is a plus
• Strong bedside manner and communication skills

Responsibilities

• Conduct in-home client assessments and reassessments
• Complete nursing visits and required documentation accurately and timely
• Observe and report changes in client condition or care needs
• Collaborate with office staff and caregivers to support quality care plans
• Provide compassionate, professional communication with clients and families
• Assist with additional nursing support as needed
