About The Position
Jefferson Health at Home by BAYADA is seeking skilled and compassionate Registered Nurses (RNs) to provide one-on-one skilled nursing visits to adults and seniors in their homes throughout Philadelphia. This unique partnership combines Jefferson Health’s nationally recognized clinical excellence with BAYADA’s mission-driven home care model, allowing nurses to deliver high-quality, personalized care where patients feel most comfortable—at home. They are especially interested in RNs who live in New Jersey, hold a Compact RN license, and are willing to work in Philadelphia, offering higher rates of pay and scheduling flexibility for eligible nurses.
Requirements
- Active RN license (Compact license required or must have PA RN license)
- Willingness to provide home visits in Philadelphia
- Minimum 1 year of RN experience
- Strong clinical judgment and time-management skills
- Compassionate, dependable, and patient-centered approach
Nice To Haves
- home health or med-surg experience a plus
Responsibilities
- Deliver skilled nursing care to adult and geriatric patients in the home
- Complete comprehensive assessments and follow physician orders
- Perform medication management, wound care, disease management, and patient education
- Accurately document care and communicate with the interdisciplinary care team
- Support patient independence, safety, and quality of life
