About The Position

The Registered Nurse (RN) provides care and services to a group of patients, visiting them where they reside, including private homes, skilled nursing facilities, and assisted living facilities. The RN is a contributing member of the interdisciplinary team and provides expertise regarding psychosocial assessments, interventions, and community resources. The RN will work in a Case Management, Admissions, or Resource role.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state or states of practice.
• Ability to work collaboratively within a team environment.
• Ability to work independently on a day-to-day basis.
• Time-management skills to manage visits, documentation, and meetings and to prioritize and triage multiple concerns.
• Ability to learn an electronic medical records system.
• Willingness to drive across the service area, with reliable transportation, a valid driver’s license, and auto insurance.
• Understanding of hospice rules and regulations.

Responsibilities

• Provide care and services to a group of patients.
• Visit patients in private homes, skilled nursing facilities, and assisted living facilities.
• Contribute to the interdisciplinary team with expertise regarding psychosocial assessments, interventions, and community resources.
• Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Manage visits, documentation, and meetings; attend required meetings and respond to urgent needs by prioritizing and triaging concerns.
• Communicate by phone, email, and in writing, and meet with healthcare professionals.
