With a team of 350+ purpose-driven professionals, we offer a compassionate alternative to institutional care through the nationally recognized PACE (Program of All-Inclusive Care for the Elderly) model. Our teams—nurses, physicians, therapists, social workers, and others—wrap each Participant in care designed to honor their choices, preserve their independence, and uplift their daily lives. As a Registered Nurse, your role reaches far beyond the walls of a facility. You’ll build meaningful relationships with older adults throughout Allegheny County, helping them remain in the homes and communities they love—where they feel safe, known, and whole. Our mission is at the heart of everything we do: to empower older adults to live independently with dignity, safety, and purpose. As an RN, you won’t just support this mission—you’ll live it every day. You’ll serve older adults throughout Allegheny County, providing care at our Day Health Centers and in Participants’ homes, community spaces, or local healthcare facilities—wherever support is needed most. This is more than a nursing job. It’s a chance to walk alongside someone in the most meaningful chapter of their life—and make that chapter safer, stronger, and full of dignity.

Requirements

  • Active PA Registered Nurse license
  • Minimum one year of experience working with older adults or medically complex individuals
  • CPR certification
  • Valid driver’s license, reliable vehicle, and auto insurance

Responsibilities

  • Complete in-depth clinical assessments and develop individualized care plans
  • Provide caregiver training on medication safety, chronic disease management, and home-based support
  • Partner with IDT members to coordinate wraparound care
  • Participate in rotating on-call coverage for clinical needs
  • Document Participant progress and care interventions clearly and consistently
