About The Position
Community Health Partnership of Illinois (CHP) is a nonprofit healthcare organization providing primary medical, dental, and behavioral health services to migrant farmworkers and other medically underserved populations throughout Illinois. We are seeking a compassionate and dependable Registered Nurse (RN) to join our Kankakee healthcare team. This position is ideal for an RN who enjoys patient-centered care, working collaboratively with providers, and making a meaningful difference in the health of the community.
Requirements
- Current, active Registered Nurse (RN) license in Illinois
- Graduate of an accredited nursing program
- Strong clinical assessment and critical-thinking skills
- Excellent communication and patient-service skills
- Ability to work both independently and collaboratively as part of an interdisciplinary healthcare team
- Strong organizational skills and attention to detail
- Commitment to providing respectful, culturally responsive care to patients from diverse backgrounds
Nice To Haves
- Previous outpatient, primary care, community health, or other clinical experience preferred
- Bilingual English/Spanish skills are a plus
- Newer RNs with strong clinical training and an interest in community health are encouraged to apply.
Responsibilities
- Assess patients and monitor changes in health status
- Obtain and document vital signs, medical histories, and other clinical information
- Provide nursing care according to established clinical protocols and provider direction
- Administer medications, immunizations, and treatments as ordered
- Assist providers with examinations, procedures, and follow-up care
- Provide patient education regarding medications, treatment plans, preventive care, and chronic disease management
- Coordinate patient care and communicate with providers and other members of the healthcare team
- Respond appropriately to patient questions and clinical concerns
- Maintain accurate, timely, and confidential documentation in the electronic health record
- Support infection-control, quality, and patient-safety practices
- Help connect patients with appropriate follow-up care and community resources
- Maintain compliance with applicable clinical, organizational, and regulatory requirements
Benefits
- Compensation: $28–$31 per hour, based on experience and qualifications.
