About The Position

A Community Admissions Registered Nurse is responsible for the presentation of Hospice services to clients who have been referred for all levels of care (general inpatient, home care, and nursing home). This role facilitates and completes the admission process after reviewing patient/family goals of care and identifying the appropriate level of care. The RN initiates the individual professional IDT plan of care and collaborates with the IDT, physician, and facility staff to determine start of care orders. Additionally, the RN provides in-services for hospital staff and the community.

Requirements

• Current Registered Nurse licensure in NY
• Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call.
• 2 years of nursing practice as a professional Registered Nurse is preferred.

Nice To Haves

• Certification in Hospice and Palliative Care Nursing preferred.

Responsibilities

• Presentation of hospice services to patients and families, individualized and sensitive to patient and family's fears, needs and goals.
• Demonstrates knowledge of guidelines for hospice eligibility and related HPCB programs, and appropriately identifies the level of hospice care.
• Performs assessments to identify all physical, psychosocial, environmental, safety, emotional and spiritual needs related to the terminal illness.
• Performs pain assessments using, adhering to pain protocols, knowledgeable in equi-analgesic dosing; optimal pain relief using an interdisciplinary focus and timely interventions.
• Able to articulate clinical course/disease progression of various diseases.
• Able to articulate etiologies of symptom manifestation related to a disease process.
• Initiates the plan of care timely and appropriately with a demonstrated skill in anticipating problems that may cause or create a threat to patient safety or lack of symptom control.
• Demonstrates knowledge in usage, indication, adverse side effects, contraindications and negative interactions of commonly used medications.
• Organizes consistently to meet productivity standards, incorporates computer documentation into patient visit and completes documentation requirements within time parameters.
• Executes patient/family/caregiver teaching in all situations.
• Establishes relationships and maintains/collaborates positive rapport with other access and hospice clinical staff and hospice support staff, hospital staff, and the medical community.
• Participates in the weekend, holiday schedule.
