Registered Nurse (RN) - Part-Time

In this role, you will collaborate with passionate individuals focused on delivering the best possible care for our clients. The Registered Nurse (RN) is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, and supervising Home Health Aides (HHAs) and Certified Nursing Assistants (CNAs). This position involves identifying, creating, and following the home health care plan for admission under a home health episode/authorization, while also educating patients, families, caregivers, and other providers on safe care practices. Our Case Management model provides timely training and support to facilitate optimal patient care. Our RNs frequently travel within either the Twin Cities metro area or the Saint Cloud metro area to visit clients in their homes.
Requirements
- Current Minnesota RN license
- Previous experience with seniors and client assessments
- Excellent customer relations and interpersonal skills
- Excellent verbal/written communication skills
- Ok with travel to and from client homes
- Possess a valid driver's license and insured personal vehicle
Responsibilities
