This position supports Lake West Medical Center Inpatient wound care needs. Hours are Monday through Friday 8-430 with some flexibility. Provides safe efficient age specific nursing care to patients/families using the nursing process in order to assist them to attain, maintain, regain, and sustain optimal health status. WOUND: · Provides consultation for individual patients with altered skin integrity (e.g. pressure injuries, draining wounds, fistulas, percutaneous tubes) to develop individualized plans of care and expected outcomes. · Provides guidance to staff in implementing protocols to identify, control, or eliminate etiologic factors for skin breakdown, including section of appropriate support surfaces. · Provides appropriate education to patients, caregivers, and staff regarding skin care, wound management, care of percutaneous tubes, and draining wound/fistula management. · Validates pressure injury data collection for nursing quality indicators via rounding, prevalence and incidents studies. OSTOMY: · Provides pre- and postoperative education to patients (and their families) who are undergoing ostomy surgery, with consideration of need for physical and psychological adaptation. · Marks stoma site preoperative by determining the appropriate sites for stoma placement with the consideration of the patient’s anatomical markings, physical capabilities and lifestyle. · Assess the stoma and stomal functioning and initiates appropriate procedure for stoma care (e.g. removes rod/statures, measures/fits a pouching system, and teaches care of the stoma /peristomal skin). · Provides appropriate education information to patients, including postoperative and discharge instruction about care, referrals, and supply needs. · Serves as a resource for patients, families, and clinicians after discharge through telephone consultation and refers to UH Stoma Clinic as needed. Provides safe efficient age specific nursing care to patients/families using the nursing process in order to assist them to attain, maintain, regain, and sustain optimal health status.
