Your expertise helps advance our mission of making communities healthier®. Our Skilled Nursing & Rehabilitation Unit includes 90 beds—20 dedicated to short-term rehabilitation and 70 to long-term care. Our interdisciplinary team includes Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), Medication Aides, and therapy partners who work collaboratively to provide high-quality, person-centered care. As a Wound Care RN, you will serve as the clinical expert for residents with wounds, collaborating closely with the Director of Nursing, physicians, and the interdisciplinary team. You will assess, plan, implement, and evaluate wound care treatment plans while supporting education, quality improvement, and best practices across the unit.

Requirements

  • Current, unencumbered RN license in the state of North Carolina
  • Graduate of an accredited School of Nursing
  • Minimum 2 years of medical/surgical experience required
  • CPR certification

Nice To Haves

  • Long-term care and wound care experience preferred
  • Wound Care certification (Preferred)

Responsibilities

  • Performs initial and ongoing wound assessments, including staging, measurements, and documentation
  • Identifies changes in healing progress and communicates findings to physicians and nursing leadership
  • Implements prescribed wound treatments and evaluates resident response
  • Participates in weekly Quality Improvement sessions and interdisciplinary care planning
  • Collaborates with nursing staff to identify resident needs and develop individualized care plans
  • Provides education to residents, families, and direct care staff regarding wound care and prevention
  • Coordinates with outpatient wound care facilities and ensures continuity of care
  • Maintains accurate, timely, and confidential documentation
  • Ensures proper infection control practices and equipment maintenance
  • Supports harmonious team relationships and promotes a positive care environment
  • Maintains clinical competency through ongoing education and training
