About The Position

Ascension Saint Thomas Rehabilitation Hospital is seeking a compassionate and skilled Registered Nurse to deliver specialized wound care services in an acute rehabilitation setting. The role includes assessing patients, providing direct wound care, educating clinical staff, and instructing patients and families to support effective treatment outcomes and rehabilitation goals. The position is full-time on the day shift at a 40-bed acute rehabilitation hospital in Nashville, Tennessee. Strong assessment, planning, and communication skills are required, along with collaboration across disciplines to ensure continuity of care.

Requirements

• Graduate of an accredited Wound Ostomy Care Nurse program.
• At least 2 years of recent wound care experience.
• Current state licensure as a Registered Nurse.
• Basic Cardiac Life Support (BCLS) certification required.
• Strong assessment, planning, and communication skills.
• Ability to collaborate across disciplines to ensure continuity of care.

Nice To Haves

• ACLS certification preferred.
• WOCN certification preferred.

Responsibilities

• Assess patients to identify wound care needs and appropriate interventions.
• Evaluate patient progress and wound-healing outcomes, adjusting care plans as needed.
• Initiate and implement nursing interventions tailored to each patient’s condition.
• Provide direct wound care based on patient condition and age-specific needs.
• Educate patients and families about wound care procedures, prevention strategies, and discharge instructions.
• Educate clinical staff about wound care treatment plans, procedures, and best practices.
• Perform other duties as assigned.
