Requirements

  • Current Arizona RN license or Compact RN License with Arizona endorsement
  • Associate’s Degree in Nursing (ADN) from an accredited nursing school
  • Minimum of 2 years clinical and Wound Care nursing required
  • BCLS and ACLS certification(s) required within 30 days of hire.

Responsibilities

  • Complete wound consults and serve as a resource to providers, staff and external contacts on issues related to wound care.
  • Assume inpatient wound care leadership and care coordination role.
  • Perform chart review of wound photographs and measurements for accuracy, bedside wound assessment, treatment plan of care coordination with facility providers, staff and administrative personnel.
  • Seek clarification of definitions, clinical diagnoses, locate source documents as appropriate and locate missing data as needed while maintaining expertise.
  • Oversee and complete Inpatient Wound Consults, including wound treatment collaboration with provider and plan of care development.
  • Work with nursing team to address pressure injury prevention protocols as appropriate.
  • Serve as point of contact for patients and providers when wound related concerns arise.
  • Provide resource assistance for discharge planning, according to current wound care protocols.
  • Responsible for provider, bedside nursing and new hire education of wound dressing rationale.
  • Identify and resolve issues affecting the delivery of wound care services.
  • Manage wounded patient lists and reports, Negative Pressure Wound Therapy equipment management and data tracking for quality assurance studies and monthly reporting.
  • Participate in Performance Improvement initiatives such as pressure injury prevention.
  • Attend regular IDT meetings to assist in the coordination of discharge planning.
  • Attend additional meetings such as hospital Fall/Harms, Pressure Injury Sub-committee, clinical team orientation and wound education.
  • May serve as a RN in the Outpatient setting to include Case Manager to a group of assigned Wound Care Center patients, as assigned and depending on the Wound Care Center setup.
  • Provide patient care to Wound Care Center patients; to include the patient intake and discharge processes as assigned and according to the hospital and nursing standards.
  • Participate in quality improvement initiatives, providing education to the patient or caregiver, and serving as a patient advocate in the delivery and coordination of the patient care.
  • Perform plan of care intervention, including medication administration, specimen collection, clinical treatments as well as other medical care treatment.
  • Monitor, record, and communicate patient condition as appropriate to care team, provider, patient, and family.
  • Collaborate as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
  • Evaluate learning needs of patient and/or family and provide patient/family education appropriate to age, culture, condition, and circumstances.
  • May also serve as Case Manager to a group of patients as assigned.
  • Perform hyperbaric oxygen therapy (HBO) related duties as assigned, upon completion of the Healogics Hyperbaric training.
  • Collaborate as needed with other healthcare providers, Wound Care Center Providers, the Program Director and the Medical Director regarding clinic and patient needs.
  • May function as a Documentation Assistant (scribe) in accordance with Healogics policy.
  • Perform other duties as required.
  • Assist in vein clinic as assigned after Healogics Vein Clinic training completed.
