About The Position

This role involves providing specialized nursing care to individuals with ostomies, as well as performing wound and skin evaluations and care. The Registered Nurse (RN), WOC Nurse Home Health, will work both independently and collaboratively within a healthcare team. Key functions include educating patients, their significant others, and staff, and precepting new staff and students in this specialty. The nurse will administer skilled nursing care in patients' residences under the direction of physician orders.

Requirements

• Registered Nurse (RN) license
• Specialized nursing care experience in ostomy, wound, and skin care
• Ability to work independently and collaboratively
• Strong communication and teaching skills
• Ability to administer skilled nursing care in a patient's residence
• Must be able to work under the direction of and in accordance with physician orders

Nice To Haves

• WOC (Wound, Ostomy, Continence) nursing certification

Responsibilities

• Provide care for individuals who have, or will have, a diverting ostomy.
• Provides wound and skin evaluation and care.
• Functions as an educator for patients/significant others and staff.
• Precepts staff and students in the specialty.
• Administers skilled nursing care in a patient’s place of residence under the direction of and in accordance with physician orders.
