About The Position

This role involves delivering holistic, person-centred care to children, youth, adults, seniors, and supporting their families. Opportunities include working in clients' homes, clinics, virtually, and engaging in community outreach. The position requires driving to clients' homes to provide care, including physical assessments, symptom control support, palliative care, rehabilitation therapy, wound care, education, and counselling.
Requirements
- Must be licensed as a Registered Nurse in the province of practice
- Must enjoy driving, have a personal vehicle and possess both a current driver's license and proof of vehicle insurance
- A satisfactory Criminal Records Check is required; a Vulnerable Sector Search and/or Child Abuse Registry Check may also be necessary
- Must be able to wear Personal Protective Equipment (PPE)
Nice To Haves
- Wound Care experience preferred
- Community health experience is an asset
Responsibilities
- Client assessments to develop individual care plans
- Provide primary care in collaboration with other healthcare professionals
- Simple and advanced wound care, including complex and negative pressure wounds
- Chronic Disease Management, e.g. diabetes care and education
- IV therapy and CVAD management
- Phlebotomy and specimen collection
- Dialysis Management
- Rehabilitative programs
- Palliative and End of Life Care
- Medication management
- Other critical nursing skills
