About The Position

Reserve Care (formerly Hospice of the Western Reserve) is seeking a Registered Nurse (RN) - Support Nurse to join their team. Requirements

  • Maintains a current Registered Nurse license in the state of Ohio.
  • Minimum of two years’ experience in hospice and palliative care preferred.
  • Must be flexible and have problem-solving skills, prioritize and demonstrate health coping strategies.
  • Ability to demonstrate excellence in written and oral communication.
  • Knowledge of hospice and palliative care clinical standards of practice, policies, and procedures.
  • Ability to practice in accordance with Reserve Care clinical standards of practice, policies, and procedures.
  • Ability to establish and maintain collaborative working relationships with Reserve Care employees, community, and healthcare providers.
  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation.
  • Ability to provide proof of eligibility to work in the United States.
  • High level of integrity, ethics, and professionalism.

Responsibilities

  • Uses the nursing process to gather information, assess, plan, implement, and evaluate patient care, addressing unique physical and emotional needs, with resources like handouts and videos available.
  • Establishes and maintains communication and collaboration with the interdisciplinary team and patient’s primary physician regarding patient condition and plan of care.
  • Anticipates needs to ensure exceptional patient-centered care.
  • Completes phone follow-up with patients who have called in over the weekend or after hours.
  • Completes phone calls from the care solutions center by fielding incoming nursing-based calls throughout the day.
  • Completes phone follow-up with patients who have been discharged from the hospital.
  • Provides phone support including questioning and noting medication changes, increased care needs, and/or changes in condition following hospitalization, verifying the next nurse visit or requesting an earlier visit as needed.
  • Communicates referral criteria based on physical symptoms, functional/cognitive changes with patients/families and refers to hospice level of care when needed.
  • Coordinates care within the agency, between transdisciplinary team members, and with external partners.
  • Ensures documentation is consistent with Reserve Care’s guidelines and paints a clear picture of the conversation and outcome of the call.
  • Contributes to and follows the plan of care in alignment with patient/family goals, collaborating with the primary nurse, social worker (SW), HNAS, SCC, and other primary team members.
  • Complies with program practices and guidelines in all interactions with team members, patients, or families.
  • Performs chart review and determines the need for an earlier visit by the primary nurse, maintaining a spreadsheet with assigned information.
  • Makes phone calls to patients/families as directed by the team for follow-up on medication changes, upcoming address changes, training needs, etc.
  • May participate in transdisciplinary team conferences, offering insights on patients discussed or those needing discussion based on phone support.
  • Educates and trains patients and families on applicable areas of hospice care, such as interventions for symptom management within the scope of practice.
  • Empowers patients and families to identify goals of care.
