Requirements

  • Current Registered Nurse (RN) license in good standing with the Commonwealth of Pennsylvania.
  • Current CPR and AED certification.
  • Strong clinical assessment, leadership, communication, and organizational skills.
  • Ability to work independently while leading a multidisciplinary team.

Nice To Haves

  • Minimum of two (2) years of long-term care, skilled nursing, rehabilitation, or institutional nursing experience preferred.

Responsibilities

  • Supervise and support nursing staff to ensure quality resident care and regulatory compliance.
  • Assist in developing and implementing individualized resident care plans.
  • Complete required clinical documentation, audits, pharmacy reviews, monitors, and other nursing office documentation in a timely manner.
  • Conduct resident assessments and monitor changes in condition.
  • Communicate significant resident status changes to physicians, the MDS Coordinator, restorative nursing, supervisors, and other interdisciplinary team members.
  • Collaborate with therapy, dietary, social services, and other departments to ensure coordinated resident care.
  • Complete admission and readmission assessments, prepare physician order documentation, and coordinate medication orders as appropriate.
  • Document resident progress, interventions, and clinical observations accurately and thoroughly.
  • Make routine rounds to monitor resident conditions, staff performance, and quality of care.
  • Respond appropriately to emergencies and provide clinical guidance to nursing staff.
  • Promote a positive, respectful environment for residents, families, and team members.
