About The Position

Matrix Home Care is seeking an experienced Registered Nurse (RN) to provide skilled nursing visits, including Start of Care (SOC), routine visits, follow-up visits, and patient assessments in the Loxahatchee, FL area. This is a field-based home care position ideal for an RN who is comfortable working independently, providing patient care in the home setting, and completing accurate and timely clinical documentation.

Requirements

Active Florida Registered Nurse (RN) license required
Must be comfortable working independently in patients’ homes
Strong clinical judgment, communication, and documentation skills required
Must be dependable, professional, and patient-focused
Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.

Nice To Haves

Minimum of 1 year of nursing experience preferred
Home health or home care experience strongly preferred
Experience with Start of Care, skilled visits, assessments, and documentation preferred
OASIS experience preferred, but not required

Responsibilities

Provide skilled nursing care to patients in their homes
Complete Start of Care (SOC), routine visits, follow-up visits, and assessments as assigned
Perform patient evaluations and monitor changes in condition
Provide patient and caregiver education
Communicate updates and changes in patient status to the clinical team
Complete all required documentation accurately and timely
Follow physician orders, plan of care, and agency policies
Maintain professionalism and compassionate care while working independently in the field
